Social Worker with a Strong Patient Focus
Do you want a job with a strong patient focus and work from a holistic perspective on the individual?
We are looking for a new colleague to join our team, as our current colleague is going on parental leave. We warmly welcome your application!
Job Responsibilities
As a social worker with us, you will be welcomed to a pleasant workplace and a team that looks forward to having you as a colleague. The work is based on the four cornerstones of palliative care – symptom control, teamwork, relationship/communication, and support for relatives to alleviate physical, psychological, social, and existential suffering. This means a strong focus on mutual interpersonal relationships, primarily with the patient but also with their relatives.
You and the team around the patient contribute to individually tailored care and consider the whole person in symptom relief. The work is stimulating and varied as it is performed in the care department and as scheduled visits in patients' homes. As a social worker, your role will primarily be to provide psychosocial support to patients and relatives. This may involve offering individually tailored support in the grieving process, crisis conversations, existential discussions, and, if necessary, providing social information and social support measures. Collaboration with other healthcare providers, authorities, and organizations is an important part of the job.
About the Workplace
The Palliative Unit is part of the East Primary Care Clinic and consists of a palliative ward, an AH team (advanced home care), and a Palliative Advisory Team that provides support to operations within general palliative care and newly established palliative reception. The clinic provides primary care in a small format by meeting the residents' needs for basic internal medicine specialist care, specialized palliative care, advanced home care (AH), qualified rehabilitation efforts, and primary care teams (NVT). The development of close care in Västmanland means that our focus is directed towards primary care and mobility, where we work from a person-centered approach. Our units are located in both Sala and Västerås.
As an employee with us, you are a significant and important part of a team consisting of doctors, nurses, nursing assistants, physiotherapists, occupational therapists, dietitians, and social workers. We work together to offer specialized palliative care that is at the forefront. We welcome you to a workplace characterized by openness and a good atmosphere, with opportunities for participation, professional development, and advantageous employee benefits.

Qualifications
We are looking for someone with a degree in social work or who is a licensed health and medical care social worker. It is an advantage if you have experience working with palliative care. If you also have experience in psychosocial work with different patient groups within somatic care, that is also an advantage. Furthermore, we see that you have good knowledge of the Swedish language in both speech and writing. A B driver's license is a requirement as home visits are part of the job.
As a person, you are empathetic, secure, and have the ability to make independent decisions in your work. This is a job for someone who thrives in collaboration with others and who wants a meaningful job close to patients and relatives. We see that you take responsibility for your tasks by structuring and driving your work forward. You are flexible and can quickly change your approach to adapt to new circumstances when the situation requires it. You communicate clearly while being attentive to the various needs of patients and their relatives. Since you will be working in a team, we see that you have good collaboration skills, relating to others in a responsive and smooth manner.
We place great emphasis on personal suitability.
